Remote patient monitoring (RPM) uses digital technologies to collect medical health data from individuals in one location and electronically transmit that information securely to health care providers in a different location for assessment and recommendations.
Physicians, Nurse Practitioners, and Physician's Assistants with a valid NPI (National Provider Identifier) who bill Medicare may participate. Many commercial plans cover RPM for in-network providers. Contact your payors to see if RPM is covered.
The beneficiaries of Medicare or similar payors who have at least one chronic condition are eligible to participate.
MPC offers care coordinators to help with onboarding patients and clinical reviews, so your staff has no additional burden.
The doctor selects based on the patient's needs. Digital Upper Arm Blood Monitor, Glucose Smart Meter, Fingertip Pulse Oximeter, and Digital Weight Scale.
MPC will send the practice a billing summary report each month by the end of the month. RPM services are reported under CPT codes 99454, 99457, and 99458 and maybe billed "incident to" under general supervision.
